This course will review the clinical management of patients with preexisting injury to their joint foundation. Three types of patients will be discussed: 1) Those with abnormal but stable joint foundations. 2) Those with abnormal but unstable joint foundations that can be rehabilitated with conservative management options. 3) Those with abnormal and progressively unstable joint foundations that require surgical management. Management options will include a review of pharmacologic agents, nerve blocks, physical therapy, occlusal splints, direct occlusal management, and joint reconstructive techniques. Insight will include how to avoid relapse from restorative, orthodontic, and orthognathic surgical treatment. Occlusal failures will be analyzed and diagnosed from direct occlusal examination, articulated study casts, cone beam CT, and magnetic resonance imaging. Selection of joint position in sleep apnea patients will be covered. Special emphasis includes stimulation of condylar development in the growing child with joint foundational damage.
