12187 (205) Under Pressure: The Management of Neonatal Shock

Abstract
Shock occurs when oxygen delivery is inadequate to meet oxygen demands resulting in organ failure. The impact of heart rate and the components of stroke volume (preload, afterload and contractility) on cardiac output will be reviewed. These concepts will be related to the different causes of neonatal shock, how each presents and their treatment. Pharmacologic treatment of shock including catecholamines, Arginine-vasopressin, corticosteroids and Phosphodiesterase inhibitors will be discussed. An algorithm detailing the treatment of neonatal shock will be presented including the concept of the 'golden hour'.

1. Discuss the pathophysiology that results in each type of shock.
2. Recognize why many infants can have a combination of more than one type of shock.
3. Review the general principles of treating shock.
4. Use pathophysiology and pharmacology to determine the appropriate combination of treatments for shock.
