The Memphis Model: Building Webs of Trust to Transform Community Health

Dr. Cutts will describe the background, structure and function of the Congregational Health Network (CHN) or “Memphis Model”.  This is a faith-partnered health system in which a hospital (Methodist Le Bonheur Healthcare) partners with over 450 churches and other community groups to improve access to healthcare and improve health and quality of life for all in our community. She will share the history, growth and current work of the CHN, as well as early health utilization and health outcome data. Lastly, she will identify critical components of CHN, so that participants can adapt parts to their work, ministry settings or clinics.
