Understanding the Patient Experience
This session will consider the ThedaCare Improvement System methodology for coordinating care transitions out of the hospital, managing patient flow within the orthopedic clinic and improving the care transition from the clinic to the hospital for patients undergoing hip replacements. Presenters will discuss how they used data from their Press Ganey Improvement Portal? to measure the impact that process changes had on patients and how they were able to improve the patient experience in those areas.ThedaCare is a 2014 Press Ganey Success Story Award winner.
