Implementing Patient Experience Improvement Strategies
The University of Kentucky Medical Center implemented Eldercare Navigation Services in May 2013 to assist aging patients with navigating the University of Kentucky Healthcare System. The service started in the emergency department and within 10 months navigation services increased to inpatients, clinic patients and discharged patients. The focus is to reduce health disparities, increase access to care and streamline care for patients.
