Implementing Patient Experience Improvement Strategies
NewYork-Presbyterian transformed seven practices in the Washington Heights-Inwood community into Level 3 NCQA Patient Centered Medical Homes. The transformation required the redefinition of front-line staff roles and a culture change from a provider-centered process to a team-based process that places the patient as the primary focus. Learn about key elements of the approach that have shown promising outcomes in the first three years of operation, including a mean Press Ganey score improvement of 2.25 points.
