Organizations must make sure discharge education is delivered to patients consistently and effectively to ensure positive health outcomes. Following an opportunity assessment, Baylor Scott & White Heart and Vascular Hospital explored the effectiveness of its discharge education process and its impact on multiple HCAHPS domains. In this session, attendees will learn how the hospital?s Patient Education Committee partnered with staff and charge nurses to provide effective discharge education. Attendees will also gain the insights necessary to evaluate their facility?s current discharge education process, engage nurses in identifying new standards for providing effective discharge education, and hardwire new behaviors through recurring audits.
