Trinity Health's People-Centered 2020 Strategic Plan is focused on building a people-centered health system that fulfills the Triple Aim of improving the patient experience of care, improving the health of populations and reducing the per capita cost of health care for the individuals and communities we serve. The organization's plan includes building new operational capabilities to deliver these outcomes as well as initiatives to transform many traditional integrated health system operations to cope with rapidly changing industry dynamics. This presentation will include a review of efforts to date, operational results, integration of these initiatives with daily operations, challenges and lessons learned.
