Effective and consistent bedside handoff communication is a critical aspect of patient-centered care delivery. A systematic review of handoff processes across INTEGRIS Health identified variations in practices and opportunities to improve the quality and consistency of handoffs across the continuum of care. Attend this session to learn how to design an effective handoff tool utilizing the EMR, conduct scenario-based training with front-line caregivers, engage patients and families in the handoff conversation, overcome barriers to adoption, and ensure ongoing sustainment of handoff at the bedside.
