Taking it to the Streets: Bringing Healthcare to the Homeless

This session examines how a group of nurses In Grand Rapids partnered with members of the homeless community to help them identify their own health needs.  Working together within the limits of their resources, they implemented unique programs to improve health and reduce the risk of chronic diseases. Sustainable nutrition interventions and a community wide walking program produced documented positive outcomes.  And through the process, community was built and people empowered.
